The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and, as a result, federal, state, and local governments and health agencies have required and recommend certain guidelines.
While Jon Lori Salon has put in place preventative measures and has taken steps to comply with relevant state and federal guidelines applicable to hair salons, Jon Lori Salon cannot guarantee that you will not become infected with COVID-19.
By signing this agreement, I hereby acknowledge the contagious nature of COVID-19 and I VOLUNTARILY, KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE ACTS, OMISSIONS OR NEGLIGENCE OF JON LORI SALON, ITS EMPLOYEES, AGENTS, REPRESENTATIVES or others, and assume full and sole responsibility for any injury to myself including, but not limited to, personal injury, illness, disability, death, damage, loss, liability, or expense, of any kind, that I may experience or incur in connection with my attendance at the salon and in connection with services received at the salon.
On my behalf, I VOLUNTARILY, KNOWINGLY AND FREELY agree to release, hold harmless and indemnify Jon Lori Salon, its employees, agents, representatives and/or other from any and all claims, including all liabilities, causes of action, damages, costs or expenses of any kind arising out of or relating to or by reason of my attendance at the salon and in connection with services received at the salon including, but not limited to, any hair, wax, and/or makeup services.
